Office hours are 8:00 am – 4:00 pm, Monday thru Friday. The office is closed on Saturday and Sunday.
Our phones are turned off for lunch between 12:00 and 1:00 pm.
Walk-In hours are from 8:30 am – 3:45 pm, Monday thru Friday. You may call the day of or the day prior to put your name in a time slot on the walk-in list. Walk-In hours are for established patients only, and limited to certain services. These services include:
- planned blood work
- planned injections such as depo provera, TDAP, flu, gardasil and lupron
- pregnancy (Beta HCG) tests
- urinary tract infections for GYN only
If you are unsure whether the service you are requesting is available on a walk-in basis, please call the office in advance.
Thank you for making an appointment with our office. We have made these forms available to you so that you can complete them and bring them with you to your initial/return visit. If you complete your forms in advance we ask that you arrive 10-15 minutes early to allow us time to process your paperwork. If you do not complete the forms in advance we ask that you arrive a minimum of 30 minutes prior to your scheduled time. Reiter, Hill, Johnson, & Nevin reserves the right to reschedule your appointment If your forms are not completed prior to your appointment time you will be required to reschedule. In addition, you MUST have your insurance card with you on the day of your visit or you will be required to reschedule.

If you are coming in for the first visit with our office for a current pregnancy, we ask that you fill out the following forms. These forms should not be be printed as double-sided because they will become part of your permanent medical records with our office.
If you are transferring care to our office during your pregnancy, our office requires that you bring all medical records pertaining to your pregnancy and that they be in English. If you are not able to provide your records in English our office will not honor your appointment.
